Definition
The mitral aortic complex is an anatomically and embryological distinct fibrous entity resulting from the fibrous union of the anterior leaflet of the mitral valve with the left and non coronary cusp of the aortic valve.
Structurally they are positioned at the crux of the heart.
Functionally they collaborate in the inflow and outflow function of the left ventricle. During diastole the anterior leaflet of the mitral valve opens and causes a relative narrowing of the LVOT while the aortic cusps get filled and distended with blood so the left and non coronary cusp pivot toward the anterior leaflet. During systole the anterior leaflet moves posteriorly to coapt with the posterior leaflet and the left and non coronary cusps are deflected away from anterior leaflet. Thus there is a pivoting of the fibrous joint with a movement toward each other during diastole and a movement away from each other during systole.
Embryologically they are closely linked in that the resorption of the subaortic conus brings the aorta into intimate contact with the mitral valve. Additionally normal flow through the mitral valve is required for the full development of the LV , aortic valve and the aorta. Poor flow through the mitral valve will result in poor development of the downstream structures of the systemic circulation typified by the hypoplastic left heart syndrome.
The close anatomic relationship and functional relationship of the mitral and aortic complexes makes disease of the one potentially cause disease of the other. This is best typified in HLHS, and IHSS for example where the subaortic stenosis causes a Venturi effect on the mitral valve, causing it to move anteriorly (rather than posteriorly) during systole, and thus causing systolic anterior motion (SAM) and preventing coaption, and thus causing mitral regurgitation.
Treatment depends on the disease and is sometimes surgical and sometimes medical.

This anatomic specimen shows the LV with septal side to the patients right and free wall side to the patients left. Note the relatively smooth walled septal surface and the papillary muscle apparatus attached to the free wall side. There are two groups of papillary muscles neither of which are attached to the septum. The anterior leaflet of the MV is in full view and note that it is in fibrous continuity with the left and non coronary cusps of the aortic valve.

Content Organization
The aortic and mitral valve are linked after the subaortic conus resorbs bringing the valves together in fibrous union. The anterior leaflet of the mitral valve is in intimate contact thereafter with the left and non coronary sinuses. Disease of the one is commonly associated with the other due to their close proximity. Clinically regurgitant changes of the aortic valve lead to changes on the mitral valve and disorders such as ASH, causes mitral valve disease to occur.
Summary
Knowledge of the embryology reflects on the anatomy and they both have relevance to the clinical and imaging aspects of these two valves provides an integrated, in depth knowledge for members of a multidisciplinary team in one comprehensive source. Each of the disciplines is treated separately and integrated into the whole module. The viewer will learn the aspects of cellular makeup, histology, anatomy, pathology, diseases, imaging, and clinical disciplines. The addition of historical events, art, and culture of the brain adds richness to the program.
