60 year old male with acute chest pain

 

Widened mediastinum >8cms, left mainstem bronchus is pushed down, trachea not shifted, no cap

High density blood behind the aorta, hemopericardium

 

SMA is ectatic suggesting arteriopathy

TypeA dissection not involving the brachiocephalic vessels but with rupture into the mediastinum and containd rupture into the pericardium No signs of tamponade. Septum shows normal bowing, IVC normal caliber

RCA not involved

RV normal size, no bowing of the septum

IVC normal caliber

Brachiocephalic vessels not involved
